Case Management

Discharge Planning

The goal of case management is to ensure a safe and timely discharge while achieving improved medical outcomes for patients admitted to ARMC. Located on the lower level of the Medical Center, the department assists patients in establishing a plan of care that takes into account the patient's initial assessment, response to treatment, reassessment and discharge planning needs. Case managers have satellite offices located near the patient care units where they are assigned.

Hours are 8 a.m. to 4:30 p.m., Monday-Friday.

Case assessment is a collaborative effort involving all disciplines, and considers:

  • Patient's age
  • Culture
  • Physical status
  • Psychosocial factors
  • Nutritional status
  • Functional status

Case managers work with a multidisciplinary team to provide comprehensive, on-going coordination of services for patients who may have multiple discharge planning needs, clinical concerns, or medical complications that require an efficient care plan for long-term management of care.

Case management services include:

  • Coordinating the delivery of services from the initial point of contact
  • Coordinating through the continuum of care
  • Discharge planning to facilitate the patient's transition from the acute care setting to an appropriate level of care upon discharge
